Some people are just blessed, you know. Everytime they have sex, the female gets pregnant even with percautions.

Sorry for being so open here, I am a medical staff. Talking about sexual organs is not something to be ashame of. I see them everyday. I’m not talking something that will arrouse you definitely.

So, when man and woman are unable to control pregnancies, they had to choose the surgical way. For females, tubal ligation. For males, vasectomy.

Let’s talk about the males.

Only brave men will choose vasectomy. I don’t know why lah. But I realize most women sacrifice their tubes instead of the men sacrificing their vas.

What is vasectomy?
Vasectomy is a minor surgery where vasa deferentia of a man are severed, and then tied or sealed to prevent the release of sperm when the man ejaculates. Two tiny incision is made. It is considered a permanent birth control because the vasa deferen is seperated, the sperm is unable to flow.

Where does the sperm goes then? The sperm are reabsorbed by the body. But it will take a few months for the man to have zero sperm count.  So if he have sex when it is not zero sperm count yet, his partner may get pregnant.

Note this! The whitish thing is not sperm. It is the semen. The sperm is mixed with the semen. So, even though the man has a vasectomy done, he still ejaculates semen. (Some girls may not know this lah.)

Maybe after some time, the man found another wife, then they wanna start another family. (I don’t know, but that could be a possibility.) Then, the man requests for a reversal of vasectomy.

Easy. Connect back the vasa deferens.
I mean, not easy. This surgery is done micro surgically, where a microscope is needed. This surgery takes way longer than a vasectomy. The incision is bigger too. The doctor will take out the testes from it’s resting place to look for the vasa deferens. When it is found, the surgeon will dilate them, ensuring there is flow by injecting some dye and lastly suture them back together. This suturing back together is called anastomosis.

The needle is super tiny, I need to strain my eyes to ensure it does not get lost.

I have not done or seen any reversal of tubal ligation in my work place. If I have a chance to do so, it would be a great experience!

Yesterday, I was scrubbing for a Lower Segment Caesarean Section. The patient was delivering her 2nd baby under General Anaesthesia. The surgeon is a very nice man, but works very fast! I learnt my speed skills from him haha!

The surgery was smooth until the baby was delivered. The baby is a healthy lil girl. Then a problem came when the surgeon had problem delivering the placenta.  The patient has Placenta Accreta.

A normal placenta seperates from the uterus spontaneouly after the delivery of the baby. In the condition where patient has placenta accreta, the placenta is unable to seperate because the placenta is abnormally attached deeply into the uterine wall. The removal of the placenta caused the patient to bleed a lot.
Of course the first thing we will do is try our best to stop the bleeding. But there were too many broken blood vessels. It was so severe, the surgeon had to do a hysterectomy (removal of the uterus). We manage to control the bleeding temporary as the surgeon went out to get consent from patient’s husband for the hysterectomy to save the patient’s life. Many colleagues came in to help because we need extra man power as we need lots of extra instruments and surgical items.
Then, when we were controlling the bleeding by stitching, we lost one needle! The blood was flooding the operating field, makes it harder to look for it. That was another nightmare! But it was when I see teamwork among my colleagues and superiors. No one was shouting but all working calmly looking for that needle. We did x-ray on patient, just incase it is somewhere in her. After the x-ray was done, we changed the drapes and we found the needle at the sticky part of the drape! *relieved for a moment*
We went on saving patient’s life. A hysterectomy is done. She is saved.
The whole theater was bloody. I used up 8 packs of gauze (80 gauzes) all soaked wet with blood. There was blood stained on my face though I wore a mask with plastic protector over my face.
What an experience. But of course I am NOT looking forward for this type of surgeries. Like watching movie hor?

I finally had time to get away from the hectic lifestyle in Singapore. At times, I feel like giving up and leave the hospital who does not really bother much about their employees who worked so hard. Oh well.

So yeah, 2 weeks break. People asked me why is that I can get such a long break? That is because I have booked my leave since last year. Yeah, we have to book  our leaves. They cannot say no because it was already approved last year. Too bad for them though. For me, 2 weeks is not enough, because I really think I need a long break from work.

Call me selfish?

I don’t think I am. Cause I’m no workaholic. Yes, money is important. But I think my health matters more. I rather take a long leave and rejunivate my body than to take MC often. I’ve been taking MCs quite often. Yes, I’m a nurse and I fall sick too often.

Many of you out there works long hours too. You get tired too. My job uses lots of physical works. We get worned out easily. We get sick easily. Sometimes we do not have time for food. So though working 8 hours a day seem ‘normal’, usually I’ll crash on my bed the moment I reach home.

Complaining much doesn’t really help. Many people did not trust my words until they see me face to face. I lost 6 kgs in total since I started work. Yes lah I happy lah I lost weight. But I lost weight in an unhealthy way. People thought I go on diet, but I actually didnt have any appetite to eat.

So now, some people even supported me giving up my bond. But I’ll try to stay until I really can’t. Or, can someone marry me? haha.

I shall be strong. I shall be strong. I shall be strong!

Take up my degree end of next year..
Take up post basic when they want to send me..
Pay back my bond (if hospital sponsor me to study OTNC (OT post basic course) and go back Malaysia if husband don’t want work in Singapore.
Take 2 years unpaid leave and study graphic designing and jaga
anak. (my husband will be struggling at work liao. HAHA!)

I need better ideas. Those are so selfish. Haha. PRAY!

Phew. It’s the weekend already. But my weekends only begins on saturday after my shift. Yes, and most of you guys begin your weekend on a friday night. I jealous. Yes. I jealous. Ending my night with Sabrina’s music from her “I Love Acoustic” album. Hit songs such as “Irreplacable“, “You and Me“, “Wake Me Up When September Ends” and “Umbrella” in acoustic. Soothing and relaxing.

This week has been yet a tiring but yet an amazing week.

Must be positive. Must stay positive no matter what happened.

Though there are things that caused me stressed, God still have been great. Well, life is good!

I am thankful for understanding colleagues and managers.
I am thankful for encouraging colleagues.
I am challenged by the management of the department and how it is runned. Many people is in the state of giving up. I’m gonna be strong and trust in the Lord when I am in that state. I pray I won’t be in that state!

A few things happened this week. But it’ll be a lil confusing (we ourselves got confused about the whole incident on how it happened) so I won’t be a story teller for that incident. Will just tell what happened briefly.

An instrument went missing after the case is over. We searched high and low, turned over all the rubbish bags, but failed to search for it. I stayed back to look for it but still failed. Went home and my colleague called me and told me it was found. Well, we do not know whether we were sabotajed, but whatever it is, God has been so good, I managed to sleep well that night!

And something funny happened today.

My duty today is 7am-4pm.
Guess what time I woke up?

LOL. I woke up at 7.27am!

Oh yes.

(Phone vibrates)
In-charge: Hello Tiffanie dear…where are you??
Me: I’M SO SORRY I OVERSLEPTT!!!
In-charge: Oh, you overslept hmm?
Me: SORRY SORRY SORRY!! I come now I come now!
In-charge: It’s ok, it’s ok. No worries dear. Take your time.
Me: I come now! I take cab!
In-charge: okok, you take cab. Don’t rush. Take your time…

And I reached work at 7.58am

And I caused come chaos at work, because I was assigned for a case at 7.30am! Haha. Last minute someone has to take my case and another who start work at 8am so happened to be there early had to start work early…ALL BECAUSE OF ME!

So I ended up doing the next case, Circumcision.
I pakar already. Anyone need circumcision?

Then, another funny thing happened. I scrubbed for a case, “Cystoscopy and Change of Double J-Stent” (A double J-Stent is a stent placed in the ureter to prevent or treat obstruction of the urine flow from the kidney).
After the case, the double J-stents that were removed were placed in a bottle to return to the patient. The surgeon was holding it as he wanted to have a look at it. Then, the team were about to transfer the patient from the operating table to the trolley. Being helpful, he placed down the bottle somewhere and helped to transfer the patient. The next thing we all know, the bottle is missing!
We searched high and low up and down side to side turned over the rubbish bags and we still couldn’t look for it!
To cut the whole story short, my colleague was shocked to find it in her pocket! Yes, how it ended in her pocket, we all have no idea! Maybe the surgeon and her forgotten that the bottle was handed over to her and she don’t even know what she took from the surgeon.
When it was found, instead of being scolded, there was laughter. Because it was funny.

Not to forget, I brought my friends to church today for a session with Dr AR Bernard’s! I’m glad they enjoyed the message. At the same time, I have received so much from tonight’s message.
